Basic policy on patient safety management
1. Our approach to patient safety
All staff, guided by our foundation's philosophy, work to secure safety and prevent medical incidents in the course of providing considerate care to each patient.
2. Committees and structure
A Patient Safety Management Committee oversees our safety framework. A Patient Safety Promotion Office carries out safety management across departments under the committee's direction, led by an appointed Patient Safety Manager. A Medication Safety Officer and a Medical Equipment Safety Officer are also appointed.
3. Staff training
Training for all staff is held at least twice a year under the committee's direction, to build awareness and knowledge of patient safety, with additional department-specific sessions held as needed.
4. Responding to an incident
If an incident occurs, our first priority is the best available medical response for the patient, followed by a prompt, honest explanation to the patient and family. An Incident Response Committee and the Patient Safety Management Committee investigate the cause and take full measures to prevent recurrence.
5. Reporting and improvement
Every department reports incidents, and near-misses, through an incident reporting system (with verbal reporting for serious or urgent cases). The committee reviews these reports, analyses patterns, develops safety measures, and evaluates their effect, sharing the results with staff through committee meetings, the safety management manual, the reporting system, and internal communications.
6. Sharing information with patients
This policy is set out in full in our Patient Safety Management Manual, which is available for patients and families to view on request, and is also published on our website.
7. Patient feedback
A Patient Consultation Desk handles concerns and suggestions from patients in good faith, and comment-form and survey feedback is used to review our service and safety practices.
8. High-risk new medical techniques
Where a department proposes a new medical technique not previously used at this institution, or a new unapproved medication, the department head applies to our High-Risk New Medical Technology Review Office, which evaluates the proposal against relevant academic society guidance before approval.
9. Continuous improvement
The Patient Safety Management Manual is reviewed and updated as needed to keep information current and improve safety.
Basic policy on infection control
Our approach to infection control
The Adventist Medical Association Foundation works to prevent and contain in-hospital infection, with every staff member acting with the understanding and cooperation of patients, and coordinating with outside institutions, medical associations, and public health authorities where a wider response is needed.
Committee structure
An Infection Control Committee, chaired by the hospital director and made up of department representatives, sets hospital-wide infection control policy. An Infection Control Team, drawn from the committee's members, carries out day-to-day infection prevention work.
Staff training
Training for all staff is held at least twice a year, planned by the Infection Control Team, to reinforce infection control practice and support ongoing learning.
Monitoring infections
Physicians report notable infection cases through a dedicated report; the Infection Control Team compiles and analyses these for the committee. Laboratory staff report positive blood cultures and multidrug-resistant organism findings; pharmacists report antimicrobial use patterns to support appropriate prescribing. Notifiable infectious diseases are reported to the public health office as required by law.
Responding to an outbreak
In an urgent case, the responsible physician reports immediately to the Infection Control Team, who work with the Infection Control Committee on immediate measures and prevention of recurrence. For emerging infectious diseases, response plans are prepared in advance in coordination with regional institutions and public health authorities.
