Our commitment to protecting personal information
Since its founding, this foundation has held to true health, in keeping with biblical teaching, as an ideal reaching beyond treating illness toward whole-person restoration. That aim depends on patients and service users providing us with the information we need.
To make it possible for you to provide that information with confidence, we recognise our strong responsibility to protect personal information entrusted to us, and commit, as one organisation, to the following.
- We maintain internal rules for personal information protection and manage personal information appropriately under them.
- We collect, use, and share personal information only within the scope needed for medical care, long-term care, and the operation of this foundation. We disclose the purpose of collection, limit use and sharing to what that purpose requires, and take measures to prevent use beyond that purpose.
- We take measures to prevent, and to correct, any leak, loss, or damage of personal information, through physical safeguards (locked storage, secured equipment), technical safeguards (access control on information systems), and organisational safeguards (rules governing information leaving the organisation).
- We provide regular training on this policy and related documents to every officer and employee.
- We maintain a point of contact for complaints and enquiries about how personal information is handled, and respond appropriately.
- We comply with relevant law, national guidance, and other applicable standards governing personal information.
- We regularly and continuously review and improve our personal information management system, with a designated responsible officer, regular audits, and ongoing improvement.

How we use patient information
Guided by our philosophy, to make man whole through Christ's love, serving each person, we aim for high-quality care and better patient service. Providing accurate, prompt care depends on information you provide us. We ask for your understanding of the purposes below, and your consent to our use of your information accordingly.
Typical purposes in medical and long-term care (based on national sector guidance)
Purposes necessary to provide your care, including internal use within this institution (the medical services we provide you, insurance claims administration, ward management, accounting, incident reporting, and service improvement) and use involving other organisations (coordination with other hospitals, clinics, midwifery practices, pharmacies, and home-care providers; responding to enquiries from other institutions; seeking outside clinical opinion; outsourced laboratory testing; explaining a patient's condition to family).
Other purposes, including internal quality-improvement analysis, staff and student training, and case review conducted within the institution, and information provided to external auditors.
A fuller, itemised list matching the national guidance template for medical and long-term care providers is set out on the Japanese page.
