What we aim for
In the spirit of Christ's own service, we serve each person with care and consideration, providing the kind of thoughtful care we would want for our own family, safely, appropriately, and as a centre this community can trust.
Our Home Care Centre supports patients moving smoothly from hospital or a care facility back to the home they know, and helps make it possible to keep living there with confidence, respecting each patient's and family's own wishes about what a good life at home looks like.
Our three services
Home Care Support Office
Our care managers listen to what patients and families want, coordinate long-term care services, prepare a care plan, and liaise with related institutions, including help applying for the long-term care certification needed to access insurance-covered care.
Visiting Nursing Station
A nurse visits your home to observe your condition, carry out medical care and medication management under a physician's instruction, and support daily living. A “home visit nursing order” from your regular physician is required. We offer 24-hour and emergency home-visit nursing arrangements, along with visiting rehabilitation from a physiotherapist or occupational therapist.
Visiting Care Station
Certified care workers provide personal care and daily living support, helping patients live as independently as possible, covering both personal care (bathing, meals, toileting) and everyday support (shopping, cleaning, cooking) under long-term care insurance.
