What we provide
Monitoring your condition
Observing overall condition and identifying a change early.
Medical care and management
Management and guidance for medical equipment (suction, urinary catheters, feeding tubes, stomas, IV lines, home oxygen), medication management, pressure-injury prevention and care, and end-of-life care.
Coordination with your physician and other professionals
Prompt contact when your condition changes, and a monthly report on your care plan.
Rehabilitation
Preventing contracture, adjusting your living environment and mobility aids, and practising going outside.
Daily living support
Personal hygiene care (bathing, sponge bathing), toileting care, support and advice for family carers, and guidance on preventing malnutrition, dehydration, and declining physical function.
A “home visit nursing order” from your regular physician is required to use this service.
