The following are the steps toward admission to our palliative care ward.

If you would like to ask about palliative care, or wish to be admitted to our palliative ward, please call our hospice consultation office. Please read our palliative care guidance document first.

2. Outpatient consultation
You will book and attend a palliative care outpatient consultation (an initial visit takes about 45 minutes). A family member or relative familiar with the patient's condition may attend on the patient's behalf if needed.
3. Admission interview
Patients wishing to be admitted to the palliative ward first have an interview at our palliative care initial consultation clinic. This clinic is fully by appointment; please book through our hospice consultation office. The office will contact you to arrange a convenient date and time. Please arrive at general reception 15 minutes before your appointment.
The initial outpatient interview is private-pay, at 5,500 yen (tax included); follow-up visits are covered by health insurance.
Please bring the following to your visit
- A clinical information document (referral letter) from your current physician, in that institution's own format
- A completed Palliative Care Ward outpatient registration form, filled in with your current physician, nurse, or social worker
- A completed Palliative Care initial consultation questionnaire, filled in by the patient or family
- The patient's own health insurance card
4. Admission assessment
Following the outpatient interview, admission registration takes place; details are set out in our guidance for patients who have completed an outpatient consultation.
5. Admission
Once a bed is available, the ward will be in touch with further guidance. Patients who have completed registration are supported comprehensively, in coordination with community hospitals and clinics.

Japan's high-cost medical care programme caps out-of-pocket spending at a monthly limit that depends on income; once registered, patients using their My Number card as their health insurance card no longer need to pay the excess up front and claim it back. Please ask our admissions office for details relevant to your circumstances.
Private room charges
- Type A private room: 11,000 yen/day
- Type B private room: 19,800 yen/day
- Type C private room: 23,100 yen/day
Private room charges are calculated per calendar day (from midnight), not per night.
Other costs
- Admission set (nightwear, towels, etc.): 880 yen/day
- Companion folding bed (with bedding): please ask for current pricing
