What does a care manager do?
We support patients in continuing to live as themselves, in the way they choose, even after being certified as needing long-term care support.
Preparing your care plan
We prepare a care plan reflecting the wishes of the patient and family, aimed at supporting a comfortable life in the home they know.
Coordination
We coordinate services covered under long-term care insurance, paid services, and related institutions, working with medical institutions and other facilities to help make everyday life as comfortable as possible.
Arranging care services
We listen to the patient's and family's wishes, coordinate long-term care services, prepare and manage the care plan, and check in on how services are working. Please don't hesitate to get in touch.

How long-term care insurance works
1. Apply
The patient or a family member applies at their local ward or city office, bringing their long-term care insurance card.
2. Home visit assessment
A ward or city official visits to assess the patient's physical and mental condition.
3. Review
Based on the assessment and a report from the patient's regular physician, the level of care needed is reviewed.
4. Certification
The ward or city certifies a care level and notifies the applicant within 30 days of the application.
5. Care plan
Once certified, a care manager prepares a service plan describing the care to be provided.
6. Services begin
Long-term care insurance benefits are generally provided as services rather than cash; patients pay 10% of the service cost themselves.

- Home help (visiting care)
- A care worker visits your home to help with housework and personal care.
- Visiting nursing
- A nurse visits under a physician's instruction to provide medical and daily-living care, coordinating with your physician as needed.
- Care management
- Preparing your home care plan and coordinating with service providers, at no cost to you.
- Home-based medical management guidance
- A physician, dentist, or pharmacist visits to provide guidance on managing your condition.
- Visiting bath service
- A mobile bathing unit is brought to your home for bathing assistance, with a nurse and care worker attending together.
- Visiting rehabilitation
- A physiotherapist or occupational therapist visits to support functional recovery or maintenance.
- Day rehabilitation
- Functional recovery training at a facility, led by a physiotherapist or occupational therapist.
- Day care
- Bathing, meals, and functional training at a day facility.
- Short-term stay
- A short stay at a care facility, or a medical facility for those needing medical supervision.
- Group home care
- A shared living arrangement, generally for around ten residents, for people with dementia who need care.
- Equipment rental & purchase support
- Rental of items such as wheelchairs or specialised beds, and reimbursement support for purchases such as a portable toilet.
- Home modification support
- Reimbursement support for small modifications such as installing handrails or removing a step.
We also support arranging care within a paid nursing home or similar residential setting.
